The Anesthesia Incident Reporting System is maintained by the Anesthesia Quality Institute, a federally listed Patient Safety Organization (PSO) based in Schaumburg, Illinois. The purpose of AIRS is to collect and analyze anesthesia incidents in order to improve the quality of anesthesia care nationwide.

All reports to AIRS are maintained as privileged and confidential within AQI’s patient safety evaluation system. Exemplary cases may be selected for educational presentation, but will not include any identifiable patient or provider information.

If you elect to 'Report Confidentially' and provide your contact information, you will be provided with a reference number for your report. This reference number can be used to review and modify your case at a later date by selecting the 'Review Previous Report' button. You can also use the reference number to automatically fill in your information on the 'Demographic Information' page when submitting future reports. You will only be contacted if further information, follow-up, or clarification is needed.

If you elect to 'Report Anonymously,' no identifying information is requested and no reference number is generated, so you will not be able to reconnect with this report.
